All Student Athletes:

1. Select the “Click Here” tab above.

2. Sign in with your parent account or click “Continue as Guest” to go straight to the forms.

3. Click on the first Electronic Form (Electronic Forms are in Blue text and are underlined)

4. Enter your student’s First Name, Last Name, Student ID, and School at the top of the form

5. Read the form and answer all questions. Some forms may include links to view additional information in its entirety.

6. After completing the form, 2 signatures are required to submit (Student & Parent/Guardian)

7. Enter a valid email address at the bottom of the form and click “Submit”

8. Complete ALL electronic forms

ALL student athletes must download and print the “Physical & Medical History” form located in the Download/Print tab or in the Athletic Office.

A new medical history and physical is required every year the athletes plans to participate in athletics.

Fill out the “Medical History” side of the form. This form must be turned in along with the physical in the upload forms. Be sure you are uploading the Physical to the Physical Upload Form and the Medical History portion to the Medical History Upload Form.
